Requirements for Iloilo City vaccination of minors

Iloilo city vaccination of minors

The Department of Health (DOH) will launch the vaccination for minors in Iloilo City on October 29, 2021.

“We really welcome this development because we really have to give protection to children aged 12 to 17 years old from COVID-19 infection,” Mayor Jerry P. Treñas said.

The venue for the inoculation will be St. Paul’s Hospital as DOH recommends a hospital-based vaccination center.

During Monday’s COVID team meeting, focal person Dr. Roland Jay Fortuna said 250 slots for pre-registered minors were identified by the hospital.

In the initial roll-out, the beneficiaries will mostly be children with co-morbidities.

Requirements to be presented at vax sites

These are the documents needed to be presented at vaccination sites:

  • Birth Certificate (Photocopy, LCR will do)
  • ID of parent (If guardian: ID of guardian, photocopy of birth certificate, photocopy of parent’s ID, authorization letter)
  • Medical Certificate from Pedia or district medical offices stating comorbidity of the child. Comorbidities include cancer, chronic kidney disease, chronic liver disease, chronic lung diseases such as asthma and cystic fibrosis, Diabetes type 1 or 2, Down syndrome, mental health conditions, heart conditions such as heart failure and coronary artery disease, or immunocompromised state such as weakened immune system and leukemia.

Meanwhile, the city will expand the vaccination for minors on November 2 to its mega sites that are in proximity of hospitals in case of any adverse reaction: Ateneo de Iloilo which is near Qualimed Hospital; SM City near Medicus Medical Center; Robinsons-Jaro near West Visayas State University Medical Center; University of San Agustin near St. Paul’s Hospital; and Megaworld near The Medical City Iloilo.

Treñas declared all the city’s mega vaccination centers will no longer require a pre-registration of minors, and will be open even for non-residents but studying in Iloilo City.

“Parents need only to bring IDs and sign the consent form onsite,” he said. (via Iloilo City PIO)
